As a Non-profit Case Manager, you will manage individuals receiving waivers and State General Fund services, maintain essential documentation, coordinate interdisciplinary team meetings, and ensure effective implementation of Service Plans. Key responsibilities include providing care coordination services, accurate case documentation, and maintaining knowledge of state and federal regulations. Requirements include a Bachelor's Degree or five years of experience in LTSS, a valid Colorado motor vehicle license, and a clean driving record.
