After you complete on one difficulty level, the game starts you from the beginning on the next difficulty but with all your skills, items, etc. intact. Same game, more and stronger enemies. Still working my way through legendary (took a break for some Fable 2/3 and Gears of War), so not sure what happens once you complete that.
