I don't think anybody beyond the original Patriots and The Boss even knew Big Boss's name. Even Miller only called him Snake or Boss.
I thought it was made pretty clear in MGS2 that he was named after the nickname for the Japanese fight plane.

Well they're both named Jack.
It depends how far back Solidus' relationship goes with Ocelot. Correct me if I'm wrong but BB's name is John, but BB tells Ocelot his name is Jack at the end of MGS3. Could be something there, I'm pretty sure Ocelot's relationship with Solidus goes back pretty far. Isn't Solidus the president on the phone at the end of MGS1?
