Detonate the red jar behind the far poison totem to destroy it. Then freeze the close totem with your axe for access to the chest.

Get to this cavern by destroying the pulsates at the mouth of the cave, from the vantage point near the artifact. Once near the chest, turn the Wheel as far down as it goes. When you release it, the pulsates will start moving, so quickly get in position to throw your axe and hit all three at once.

Rare Talisman

A last-second dodge activates Realm Shift, temporarily slowing down surrounding enemies. Upgrading this item increases the duration of Realm Shift.

Heavy Runic Attack

3 powerful slam attacks that launch enemies and then sends them flying.

From the northern room, throw your axe at the pulsate that sits just above the chest. Move back out to the room with the door mechanism and stand in front of the gate with the chest. Recalling your axe from there should destroy both pulsates and open the gate.
